I understand that:

  1. It is the intent of the Moscow Youth Football Association to deny participation to any person who has been convicted of a crime of physical violence or sex-related offense against a person. 
  2. One-on-one interactions between a minor and an adult (who is not the legal guardian) shall be limited to those that are observable by another adult and within an interruptible distance, except under emergency circumstances. 
  3. It is required to report any known or suspected abuse incidents to law enforcement. 
  4. In applying for a Moscow Youth Football Association position, the information which I have furnished on this form is subject to verification, which may include a criminal history check. 
  5. This registration/disclosure statement must be updated every year.

I will place the emotional and physical well-being of my players ahead of any personal desire to win. 

I will remember to treat each player as an individual, remembering the large spread of emotional and physical development for the same age group. 

I will do my best to provide a safe play situation for my players. 

I will lead by example in demonstrating fair play and sportsmanship to all of my players. 

I will ensure that I am knowledgeable in the rules of the sport that I coach, and that I will teach these rules to all of my players. 

I will use those coaching techniques appropriate for each of the skills that I teach. 

I will remember that I am a youth coach, and that the game is for children and not adults. 

I will demonstrate sportsmanlike behavior towards players, referees, coaches, and fans. I will have an expectation of sportsmanlike behavior of my players at all times.
